Please fill in all required information. A copy of your responses will be sent to the email you provide, which will also serve as confirmation of your request for care.
A member will contact you within 3-5 business days to advise you if we have a spot on one of our teams, or if you have been placed on our waitlist.
IMPORTANT WAITLIST INFORMATION
If you are placed on our waitlist, our staff is unable to provide information about how many people are before you on the list, as it is constantly changing. Please rest assured that we want you to have a midwife too – we’ll be sure to contact you as soon as possible if a spot becomes available.
